Fill out COVID-19 Immunization Consent Form-- you may also fill this out at the Pharmacy when you show up for your appointment.

Please be aware that vaccine supply is limited, and it is possible that our pharmacy may run out of vaccine at any time. We will do our best to notify you if your appointment needs to be rescheduled. Your patience during this time is greatly appreciated.
